Online Parent Information Form

On the first day of school your child will be bringing home with them a cluster information sheet asking you to fill in contact information so that the cluster teachers can reach you to discuss your child's progress or update you about their classes. This is located on the back of the "Million Word Assignment".

This year I would like to compile this information from you electronically. It will allow me to get your contact information (email addresses, phone numbers, etc.) to your child's teachers in a more accessible manner. Rather than filling out the hard copy of the form I would suggest you click on the BEGIN YEAR PARENT INFORMATION FORM link at the top of this blog. You will be redirected to a google form I created to compile your contact information. If you fill out the form online you do not need to fill out the hard copy and send back with your child.
