First Day of School Forms

Hello all,

  We hope your child returns home from school today excited and ready to learn and be a member of the 8th grade, our cluster, and all of our classes. With the first day of school comes forms which must be reviewed and returned to school. Your child's homeroom teacher has sent home a folder of school forms from the main office that should be returned by this Friday. In the folder you will find three papers:

1. Student Biographical Information Verification Report - Please review the information on this form and make any changes on the form necessary. Please sign/initial the sheet as well

2. Medical information form - this is located on the back of the Biographical Information Report. Please fill this out completely so that the school nurse has the most up to date medical information regarding your child in the event of an emergency at school.

3. Cluster Information Sheet/"Million Word Assignment" - The cluster information sheet can be filled out online (preferably) by clicking on the Begin Year Parent Information form at the top of this blog. This will allow the team of teachers to easily get in contact with you regarding your child's progress in their respective classes. If you fill out the form online on this blog then you do not have to fill in the paper version

The "Million Word" Assignment is an opportunity for you to share with the cluster teachers a bit about your child as a student, individual, person, family member. You can either return this through your child's homeroom teacher or email your "assignment" to richard_battaglia@wayland.k12.ma.us
